HEALTH INSURANCE PLANS
We are preferred providers for most health insurers and most insurance companies provide coverage of physical therapy services. As a courtesy to you, our staff will verify your physical therapy benefits prior to your first appointment. Verification of benefits is not a guarantee of payment. You are responsible for your insurance coverage and payment on your account. We encourage you to learn the requirements and restrictions of your particular plan. Common requirements and restrictions of some plans include:
- Referral from a primary care physician
- Co-payment at time of treatment
- Annual monetary or number of visit cap on services
- Deductible responsibilities
We file insurance claims directly to your insurance company. Co-payments are due at the time of treatment and our front office staff will be happy to answer any questions you may have regarding your account.
CASH PAYMENT ~ NO INSURANCE
At Fischer Physical Therapy we believe that no one should be denied physical therapy due to lack of insurance coverage. Our clinic offers a discounted cash rate to those who do not have appropriate insurance coverage. Payment will be required at the time of service unless other arrangements have been made. WORKER’S COMPENSATION
If you were involved in a work-related accident or injury and have a current worker’s compensation claim, you may be eligible for physical therapy. Most worker’s compensation companies require a physician referral for physical therapy and approval must be granted by the insurer. We currently accept all patients requiring rehabilitation for a work-related injury.
MOTOR VEHICLE ACCIDENT
We work with all individuals who have been involved in an auto accident. In most cases, the auto insurance company will request that all medical services be billed to the auto insurer of the vehicle the patient was in, not necessarily the vehicle at fault. We are willing to directly bill claims to which ever insurance is decided upon by the patient and their auto claim manager. Physical therapy services as the result of an auto accident typically require a referral from a physician prior to beginning rehabilitation.
